Register with the Practice?

The practice has a dedicated new patient registration desk which is open to accept new registrations during practice opening hours.

Registering with Queenswood Medical Practice is straightforward and easy.

  • You complete a New Patient Health Questionnaire
  • You complete a registration form
  • You provide your NHS number or verify you are entitled to NHS treatment

For your convenience you will find below a registration form and New Patient Health questionnaire which you can fill out and email into the surgery on [email protected] This will speed up the registration time when you come into the surgery.

All patients will be allocated a named GP.

If you do not wish to have a summary care record, please complete an opt out form and bring this with you when you are registering. You can obtain an opt out form by following this link

If you decide to come directly into the surgery without completing the above forms please go to the Registration desk on the first floor reception and someone will be glad to help you go through the registration forms.

You will be given a form to complete and you will be asked for proof of your address by showing us a recent official document such as a bill or an official letter. This verifies that you live in our practice area.

We may also ask for additional proof that you are entitled to NHS treatment before we can accept you therefore you may need to provide your NHS number and details of your last doctor. If you do not have an NHS number we will require you to provide accurate details of your last doctor.

If you cannot remember, we will be unable to complete your registration until you have that information in your possession. The reason is that without details of the last doctor holding your registration, we may not be able to gain access to your previous medical records, and this may compromise the treatment we can give you.

Following this procedure, we may ask you to attend a registration clinic to be seen by a nurse or healthcare assistant to have some aspects of your present health status confirmed and checked.
