Register with Us

Registering with Queenswood Medical Practice is straightforward and easy. Please firstly check that you live within our catchment area. You will then need to:

  • Complete a New Patient Health Questionnaire
  • Complete a registration form
  • Provide your NHS number or verify you are entitled to NHS treatment

All patients will be allocated a named GP.

If you do not wish to have a summary care record, please complete an opt out form and bring this with you when you are registering. You can obtain an opt out form here.

If you decide to come directly into the surgery without completing the above forms please go to the Reception desk on the first floor reception and someone will be glad to help you go through the registration forms.

You will be given a form to complete and asked to verify that you live in our practice area. If you do not have a fixed abode or are staying temporarily in the area, inform the receptionist of this and your registration will still be accommodated.

We may also ask for additional proof that you are entitled to NHS treatment before we can accept you, therefore you may need to provide your NHS number and details of your last doctor. If you do not have an NHS number we will require you to provide accurate details of your last doctor.

If you cannot remember, we will be unable to complete your registration until you have that information in your possession. The reason is that without details of the last doctor holding your registration, we may not be able to gain access to your previous medical records, and this may compromise the treatment we can give you.

We would usually invite you into the practice to check your height, weight and blood pressure. Currently, due to the COVID-19 pandemic we are unable to do this. We aim to complete all registrations within 3 working days. If there are any queries with your registration this may take longer. Please contact us after 3 working days to see if your registration has been completed.
