National Data Opt-out

A new national data opt-out model launched in May 2018 to provide an online facility of individuals to opt out from the use of their confidential data for purposes such as research and health service planning.

Patients can set their opt-out preferences at https://www.nhs.uk/your-nhs-data-matters/ You will need your NHS number and a valid email address or telephone number.

Patients who are unable to use the online facility can use a phone helpline to manage their choice –0300 303 5678. A paper print-and-post form is also available at https://www.nhs.uk/your-nhs-data-matters/manage-your-choice/other-ways-to-manage-your-choice/

If your query is non-urgent, please visit www.nhs.uk and use the symptom checker for further advice on self-care and how to get the right help. If you are experiencing severe chest pains, breathing difficulties or require urgent medical attention, call 999. Please avoid coming in to the surgery unless asked to do so as we continue to prevent the spread of coronavirus. Thank you for your patience and understanding.
