Register with our surgery online
We are using an NHS online service called Register with a GP surgery that makes it easy to register with this GP surgery.
Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.
The service is designed and run by the NHS, so your personal information is safe. It reduces our administrative workload and makes it easier for you to register.
Paper forms are still available if you need one.
Online Access
If you would like the facts or the forms needed to either get your prescriptions delivered straight to your pharmacy of choice, or to get online access to your records, where you can book nursing appointments online, order prescription, or get your test results.
Named Accountable GP
You will be assigned to a doctor who will be your “Named Accountable GP” as required by the NHS, however, you can choose to be seen by any clinician as long as they are available.
You will be added to the practice list, not an individual doctor. You can ask to be seen by any of the doctors in the practice although it will not always be possible to see the doctor of your choice on a particular day and time.

The National Care Record Service (NCRS) is the way in which health professionals now access the Summary Care Record (SCR). The SCR has not changed, it is just the way SCRs are accessed that has changed.

Organ Donation
